New Client Forms & Payment

A link to the Client Portal will be provided to you in order to complete necessary paperwork prior to your initial appointment.  If you prefer to download the paperwork, please click on the link below. After you read and print the forms, please fill out and sign them. Retain copies for your records. Please bring the completed and signed forms to your first visit.

It is my intention to provide services in a professional manor consistent with accepted ethical standards of practice. Regular counseling sessions are usually held weekly and will be for approximately 50 minutes. It is impossible for any psychotherapist to guarantee any specific results regarding your counseling goals. However, together we will work to achieve the best possible results.

Payment Information:

Payment is required in advance or at time of service. Additional sessions will not be scheduled if a balance is outstanding. Accepted Payment Methods are Cash, Mastercard, Visa, HSA, American Express. Contact Carla J. Wynn if you have any questions
